Traumatic pancreatic injury is a rare but potentially fatal condition, accounting for 0.5-8% of abdominal trauma cases. Early diagnosis and assessment of pancreatic duct integrity are crucial, as ductal injury is the primary predictor of morbidity and mortality, with an overall mortality rate reaching 30% when present. We present the case of a 12-year-old girl with closed abdominal trauma who developed a pancreatic ductal injury. A pancreatic stent was placed, but intra- and peri-pancreatic fluid collections subsequently developed, requiring endoscopic drainage with a double pigtail stent. After three months of follow-up, with significant clinical, laboratory, and imaging improvement, the stents were removed. The patient remains follow-up without complications.
