Small bowel adenocarcinoma is a rare neoplasm, accounting for less than 2% of all gastrointestinal malignancies. Its diagnosis is frequently delayed due to nonspecific clinical presentation, contributing to a poor prognosis. Although it has been associated with predisposing conditions such as Crohn’s disease, its pathophysiology remains poorly understood. We report the case of a female patient presenting with chronic abdominal pain and significant weight loss, in whom a small bowel adenocarcinoma was diagnosed after multiple diagnostic evaluations. Management involved surgical resection, and histopathological analysis confirmed the malignancy. This case underscores the importance of including small bowel adenocarcinoma in the differential diagnosis of chronic abdominal conditions to facilitate early detection and improve clinical outcomes.
