Three-dimensional post-processing (3D) of colonic tomographic images was applied in two patients with uncertain diagnoses. In the first case, the presence of colo-colonic fistulas was suspected, while in the second, a giant solitary sigmoid diverticulum was considered. Surgical treatment was initially indicated for both patients. After further evaluation with 3D post-processing, the preliminary diagnoses were excluded, the true underlying causes of symptoms were identified, and unnecessary surgery was avoided. These cases highlight the clinical value of 3D tomographic image post-processing as a complementary tool for assessing complex scenarios in which precise delineation of colonic anatomy is essential.
