Introduction:

Endometriosis consists of the implantation of endometrial tissue outside the uterus, affecting up to 10% of women of reproductive age. Its deep form can compromise urinary, respiratory, and gastrointestinal organs, with gastric localization being extremely rare. Clinical presentation depends on the site of implantation, often mimicking functional digestive or urinary disorders.

Case Report:

A 42-year-old woman presented with a 7-year history of burning epigastric pain, early satiety, and intermittent postprandial bloating, related to the menstrual cycle. No relevant clinical findings were noted. Previous endoscopic studies had revealed severe erosive gastroduodenitis and infectious colitis. Due to persistent and worsening symptoms, a new gastroscopy was performed, showing erosive pangastropathy and a reddish lesion in the gastric body. Biopsy revealed hemosiderin deposits and endometrial glands with clots, consistent with endometriosis.

Conclusions:

Endometriosis is common, but its presentation in the gastrointestinal tract poses a diagnostic challenge due to its similarity with functional disorders. The presence of cyclic symptoms should raise suspicion of this condition. Timely diagnosis guides therapeutic decisions, including surgical options in cases of deep endometriosis.
