Solid pseudopapillary tumors of the pancreas are rare neoplasms with usually benign behavior and favorable prognosis. They are often discovered incidentally, as they may remain asymptomatic for long periods. Histologically, they present a mixed solid-cystic pattern without mucin. Surgical management ranges from enucleation to distal pancreatectomy, depending on tumor characteristics.
Case Report:
A 21-year-old female presented with a 3-year history of left flank abdominal pain, exacerbated by Valsalva maneuvers. She had no relevant medical history and a normal physical examination. Upper endoscopy revealed chronic gastritis. Contrast-enhanced CT showed a well-defined, heterogeneous, rounded lesion (7.7 cm) in the pancreatic tail, with preserved cleavage planes. She was referred to hepatobiliary surgery and underwent distal pancreatectomy with splenectomy. The specimen revealed a 15 cm mass involving the splenic hilum. Histopathological analysis described a well-differentiated lesion with solid and papillary areas, alveolar architecture, intact capsule, and no vascular invasion, consistent with a solid pseudopapillary tumor.
Conclusion:
Despite its size, this rare pancreatic tumor demonstrated benign behavior. Imaging findings facilitated accurate diagnosis and timely surgical management.
