A case of a fetus with acardiac and acephalus, product of twin pregnancy complicate with reversed arterial perfusion syndrome, whose diagnosis was suspected by ultrasound found acephalus and lack cardiac activity, in a monocorial twin pregnancy and to be confirmed with the application of Doppler ultrasound. The perinatal management consisted in the realization of fetal echocardiographic and pulsated-wave Doppler ultrasound in periodic shape, how resort by to check ad to predict the degree of cardiac descompensation of healthy fetus, to permit from the point of view clinical the possibility of to determine the opportune moment for the delivery and to guarantee the viability of healthy fetus, whose evolution postnatal was satisfactory
