SUMMARY Objective: To establish a hemodynamic Doppler profile as a method of antenatal fetal well-being. To evaluate the efficiency of the fetal biophysical profile and hemodynamic Doppler profile to predict acidosis and/or fetal morbidity. Method: Prospective study of 41 high risk pregnancies, delivered by cesarean section, from April of 1999 to March of 2000. Setting: Perinatology Unit "Dr. Freddy Guevara" at the University Hospital of Caracas. Results: The average gestational age was 37.2 weeks. The fetal biophysical profile was abnormal in 56.1% cases (n = 23), while the hemodynamic Doppler profile was in 61.0% (n = 25). Of the newborns, 13 (31.7%) had acidosis, and 15 (36.6%) restricted growth. The hemodynamic Doppler profile was better than the fetal biophysical profile to predict fetal acidemia and perinatal morbidity. The logistic regression analysis of point scale demonstrated than with a low score the severity of the acidemia was worst. Conclusion: The hemodynamic Doppler profile demonstrated to be a more efficient antenatal method to predict fetal acidemia and morbidity, nevertheless, we recommend the combine use with fetal biophysical profile
