Objective: To know the casuistic, evolution and management of the dead twin syndrome. Method: Retrospective and analytic study of 1 592 cases of patient with twin pregnancies, with one of the fetuses dead in the period 1984 - 1999. Setting: Obstetric Division of the University Hospital of Maracaibo, Maternity "Dr. Armando Castillo Plaza" Maracaibo. Venezuela. Results: There were 37 cases with one of the fetuses dead. Incidence 2.32 % corresponding to 1 case for each 43 twin pregnancies. Mean maternal age 23 years, 43.24% primigravida; 61.76 % preterm 61.7 % controlled; 62.16 % of the births were normal deliveries. The placenta was monocorial-biamniotic in 7 cases (36.84 %). The Apgar at 1 minute was 8 to 10 points in 54.05 %, with 2.9 % of congenital malformations. Maternal morbidity 11.7 % and cero mortality. Conclusions: It is necessary the evaluation of each pregnancy in order to prevent the maternal fetal and neonatal complications of this syndrome. The childbirth should be vaginal, with study of membranes and placenta.
