SUMMARY Objective: To study patients that leave against medical advice, related factors, to establish causes and to make suggestions for a more effective obstetric attention. Method: Analytical, epidemiological, retrospective and descriptive study of 205 cases happened during July 1999 to July 2001. Of 6 692 obstetric discharges there was an incidence of 3.06 %. Setting: Department of Obstetric and Gynecology, "Dr. Adolfo Prince Lara" Hospital, Puerto Cabello, Carabobo State. Results: The patients proceeded 57.07 % from neighborhoods; 22.91 % lived in the Estado Falcon and Yaracauy, 37.07 % were concubines and 32.19 single; 38.14 % had between 20-24 years ad 25.85 % 19 year-old or less, and 69.28 % didn’t control the pregnancy. In family records highlighted the arterial hypertension (28.98%) and diabetes (26.81 %); in personal records the previous cesarean operation (19.83 %) and miscarriage (19 %); and in the admission diagnosis, the abortion (50.24 %), labor (13.65 %) and to retain of remains (8.78 %). 59.51 % was high gravity, 70.73 % pregnancy age 22 weeks and less, and in 73.17 % were carried out uterine curettage. The most frequent months were July-August 34.63 % and November-December 27.80 %; the day of the week Friday 28.78 % and Saturday 28.78 %; the hour 7 a.m.-1 p.m. 53.65 %. Conclusion: Behind the leave against medical opinion hides mainly the abortion with all their facts and family, educational, social, morals problems, everything forces to double integral efforts.
