Objective: To evaluate the efficacy in decreasing the frequency of post operative associated shoulder pain using low intrabdominal pressure during the setting of pneumoperitoneum. Methods: Randomized clinical trial with thirty patients undergoing laparoscopy were randomized in two groups. Group A (experimental group) and Group B (control). Interventions: Gynecology laparoscopy for benigns conditions. Group A surgery performed at low intrabdominal pressure (8 mmHg of pneumoperitoneum) and Group B underwent surgery with the usual preset intrabdominal pressure for laparoscopy (15 mmHg of pneumoperitoneum). Settings: private urban obstetric and gynecology clinic in Merida, Venezuela. Results: Shoulder tip pain was assesed 24, 48 and 78 hours after surgery. We found in the first 24 hours an occurrence of shoulder pain in 13% of the patients of group A and 53% of group B. Difference between groups were statistically significative p<0,05 C.I.(0,012-0,973) and reduction of relative risk was 75%. No diffference were found at 48 and 72 hours after surgery. There was no increase in time of operation, blood loss or complication in the experimental group. Conclusion: Performing gynecology laparoscopy with low intrabdominal pressure (8mmHg of pneumoperitoneum) is safe and effective in reducing de frequency of post operative associated shoulder pain.
