Objective: To evaluate a modification of the technique of Pfannenstiel used in the caesarean section. Method: Prospective study of 57 cases of the private patients, to whom the modified operating technique were practiced. Setting: Centro Clínico de Maternidad "Leopoldo Aguerrevere". Results: The age average was 30.46 years; 16 cases (28%) were previous Cesarean section. The indications of cesarean section were in 22 cases (38.5%) the presence of cesarean uterine scar and in 13 cases (22.8%) fetuspelvic disproportion. In 56 cases (95%) the fetal presentation was cephalic. The fetal weight average was 3 038 g and the height average of 48 cm. The surgical time average was of 1 hour 18 minutes, and there were no complications inherent to the technique. Conclusion: This modification extends the operating field, improves the extraction of the fetal pole, without prolonging the surgical time and giving an excellent aesthetic result.
