Objectives: To know the maternal and perinatal mortality rates and their tendencies during 36 years of medical care. Methods: Descriptive, retrospective and epidemiological study of 115 direct maternal deaths, 5 236 perinatal deaths: 2 856 fetal deaths and 2 370 neonatal deaths; in 126 669 newborns from 1969-2004. Setting: Hospital Dr. Adolfo Prince Lara, Puerto Cabello, Estado Carabobo, Venezuela. The percentage variations, line of logarithmic tendency and its equation were determined. Results: The maternal mortality rate was of 99.2 per 100 000 newborns or 1 per 1 100, in the last ten years 48.2 per 100 000 or 1 per 2 075; the rates display variable values, with tendency to drop throughout the period, stabilizing at the end, reduction of global percentages variations 82.64 %, with ranges from 286.3 to 21.5 %; global logarithmic tendency line decreased. The perinatal mortality rate was 41.9 per 1000 newborns, fetal mortality 23.4 and neonatal 18.5; there were extreme numbers of 59.3 (1975) and 276 (1987), with tendency to decrease during the period, the reduction of global percentage variations 8.39 %; global line of logarithmic tendency to the reduction. Conclusion: Maternal mortality reveals median numbers, with tendency to drop and stabilize at the end; perinatal mortality showed high values and tendency to drop, with predominance of neonatal mortality. Everything indicates the need to reinforce the preventive plans to lower these important health indicators.
