Objective: To study the preterm birth, its obstetric conditions, as well as to describe the neonatal state and its morbidity-mortality. Setting: Department of Obstetrics and Gynecology, Hospital Dr. Adolfo Prince Lara, Puerto Cabello, Estado Carabobo. Methods: A retrospective and descriptive epidemiological analytical study of 412 premature newborns, during 2003-2005 period. 7940 were born alive in this two year period, which means an incidence of 5.18 %. Results: The actual predominant pathology was the premature labor 32.43 % (131/404), threat of premature labor 21.79 % (88/404), the premature membrane rupture 15.60 % (63/404), pregnancy induced hypertension 10.89 % (44/404). The 59.65 % have had previous pregnancies (241/404), with 59.9 % (242/404) with 31 and 36 weeks getstion, and 66.09 % (267/404) vaginal delivery. The newborn were female 50.49 % (208/412), weight between 2000 and 2499 g 33.01 % (136/412), height 45-49 cm 37.86 % (156/412); Apgar index 6 or less 84.12 % (265/315). The global morbidity was 26.45 % (109/ 412), and global perinatal mortality 40.05 % (165/412), fetal mortality 23.54 % (97/412), neonatal mortality 16.5 % (68/412), respiratory distress syndrome and sepsis were determining factors. Conclusion: Prematurity still reveals a high rate of perinatal morbidity-mortality, contributed mainly by the respiratory distress syndrome and sepsis. Everything confirms that the improvement of preventive programs and neonatal care is essential. It must be considered a high priority in our investigations.
