Objective: To evaluate the effectiveness of metformin in the modification of the ovarian function and cardiovascular factors of risk, in patients with polycystic ovary syndrome and hyperinsulinism. Methods: Experimental and prospective study in patients who consulted between August 2005 and July 2006 with diagnosis of PCOS and hyperinsulinism; the cardiovascular modifications in the hormonal profile, risks and Doppler pattern were determined subsequent to the treatment with metformin with doses of 1 500 mg/day. Setting: Servicio de Fertilidad de la Maternidad "Concepcion Palacios" Results: There was a significant reduction of weight, BMI, WHR, BP, concentrations of LH, prolactin and androgens when culminating the 3 months with metformin (P< 0,05), with significant improvement of the lipid profile but did not generate great changes in the categorization of the global cardiovascular risk. The ovarian RI initially presented high values that descended subsequent to the treatment. The WHR presented direct and significant correlation on ovarian RI, while the corporal weight, BMI and glucose 120 min influenced on the blood pressure. Conclusion: One is due to consider to metformin as a therapeutic alternative for the integral handling of the patient with polycystic ovary syndrome.
