Objective:

To develop a valid and reliable tool to evaluate clinical performance in postpartum hemorrhage management.

Methods:

The tool was evaluated in the context of simulated scenarios. Its main objective is based on the training and training of the perinatal rapid response team, as an interdisciplinary shock instrument in the obstetric emergency. A Delphi panel reached consensus on a) Elements to include in the evaluation tool, b) Weighting of each element, and c) Final Tool Formation. Validity and reliability were evaluated according to Cook's method. After theoretical training, they began simulation scenarios using medium-fidelity environments. The strategy of hot analysis was applied for feedback on each case. Random reassignment of roles and re-evaluation of the group was carried out through a new clinical case. For the final evaluation, the work teams were randomized and the skills acquired strengthened.

Results:

A total of 459 participants were evaluated, distributed in 36 working groups. Teams had an average score of 51/100 (95% CI: 44.47-57.53) in year 1; 76/100 (95% CI: 69.51-82.58) in FY 2 and 84/100 (95% CI: 77.42-90.56) in the final assessment (p=0.00001). Significant improvement in the clinical performance of perinatal rapid response team was recorded with the use of this tool (p<0.05). The tool was applicable in all simulated scenarios (ICC 0.98).

Conclusion:

The tool appears to be valid and reliable for evaluating clinical performance in simulated environments and is recommended for use as an perinatal rapid response team guide in real postpartum hemorrhage scenarios.
