Resumen

SECO, Maryem; MONSALVE, Nazira  y  NOGUERA, María Eugenia. Evaluación histeroscópica posterior a legrado en la enfermedad trofoblástica gestacional. Rev Obstet Ginecol Venez [online]. 2020, vol.80, n.3, pp.215-226.  Epub 15-Oct-2020. ISSN 0048-7732.

Objective:

To describe the hysteroscopic findings after the first curettage by Manual Vacuum Aspiration in patients diagnosed with Gestational Trophoblastic Disease (GTD), in the Department of Obstetrics and Gynecology of the Autonomous Institute of Los Andes University Hospital, between May 2016-May 2018.

Methods:

Observational, cross-sectional, mixed study to evaluate the uterine cavity by hysteroscopy in patients with GTD that meet the inclusion and exclusion criteria.

Results:

A sample of 30 patients with an average age of 30 ± 11 years was obtained. Genital bleeding was the main reason for consultation (70%) and the weeks of amenorrhea were between 7-18.2. The median of human chorionic gonadotropin-β (βhCG) before curettage was 2548 ± 1288 mUI / L and later of 285 ± 427 mUI / L (P= 0.042). Hysteroscopy reported the presence of placental polyps in 22 of 30 (73.3%) patients and in their absence (26.7%). Of 21 cases that the transvaginal ultrasound concluded present ovular remains were corroborated by hysteroscopy in 16 of them their presence, and only 3 cases of 9 corresponded to the true negative transvaginal ultrasound. The transvaginal ultrasound to evaluate ovular remains in comparison with the office Hysteroscopy presented a sensitivity of 72.7% and a specificity of 37.5%.

Conclusion:

The office hysteroscopy should complement the transvaginal ultrasound and βhCG titers in the follow-up of the GTD.
