Spontaneous rupture of mature cystic teratoma of the ovary is rare, but it can lead to the appearance of acute chemical peritonitis. Once diagnosis is made, it is necessary to perform surgery immediately. Tumor resection and peritoneal lavage are generally sufficient to reverse effects. We present a case of a 35-year-old patient who presented with symptoms of acute abdomen secondary to acute chemical peritonitis of two days of evolution. Emergency laparoscopy was performed with approximately 1000 mL of yellowish, non-fetid ascitic fluid in peritoneal cavity and cystic tumor originating from the left ovary, performing left salpingo-oophorectomy and peritoneal lavage. The anatomopathological evaluation confirmed the diagnosis of mature ovarian cystic teratoma.
