Abdominal ectopic pregnancy has a low incidence of 1 per 10,000 pregnancies, accounting for 1% of ectopic pregnancies. It is classified as primary or secondary abdominal ectopic according to its established genesis. There is a 35-year-old patient, IIG AI, with a history of tubular ectopic pregnancy diagnosed at 8 weeks gestation, unresolved, who eight weeks later consulted for pain and bleeding. She was admitted with a diagnosis of abdominal pregnancy, with ultrasound showing fetus in abdominal cavity and suggestive image of hematoma covering the posterior region of the uterus, right ovary, omenta, small intestine and transverse colon. Infraumbilical media laparotomy was performed with fetus removal, left salpingectomy, trophoblastic tissue dissection, omentectomy and appendectomy. Subsequently, medical treatment with methotrexate was indicated, with discharge under satisfactory conditions and control by external consultation.
