Angiomyofibroblastoma is a benign, nodular, well-circumscribed, rare, benign mesenchymal lesion located in the vulvovaginal area. The appearance as a pedunculated polypoid tumor is rare. A case of vulvar pedunculated pedunculated angiomyofibroblastoma is presented in a 37-year-old female patient who presented with a painless pedunculated tumor of vulvar origin. A polypoid, pedunculated, painless tumor was observed originating from the right labium major. It was decided to perform a simple resection of the totality, finding a homogeneous, yellowish, smooth tumor. Immunohistochemical staining showed that the stromal cells were immunoreactive for vimentin, while some of these cells were also positive for desmin, estrogen and progesterone receptors. Vulvar pedunculated angiomyofibroblastoma is a benign entity whose correct diagnosis is essential since it shares clinicopathologic, immunohistochemical and histogenetic features with probably other tumors. This diagnostic possibility should be considered in patients with vulvar tumors in order to achieve an accurate treatment.
