High-grade serous papillary cystadenocarcinoma with omental metastasis is a rare and aggressive type of carcinoma. This report describes the case of a 43-year-old woman with a family history of cancer and a history of infertility, who presented with persistent pelvic pain. Transvaginal pelvic ultrasonography revealed a multiloculated lesion in the right ovary, classified as O-RADS 5, suggesting malignancy. Computed tomography confirmed a solid right adnexal mass with moderate ascites. During surgery, excision of the ovarian tumor and the uterus was performed, confirming high-grade serous papillary cystadenocarcinoma with focal metastases in the omentum. Optimal management of this cancer requires a multidisciplinary approach and rigorous clinical surveillance.
