Genital ulcers are a frequent cause of gynecological consultation. Although most are caused by pathogens considered sexually transmitted such as those caused by herpes virus, syphilis, soft chancre or granuloma venereum, there are also non-infectious ulcers due to other entities that can cause repercussions on the patient and her family and social environment. We present the case of a patient who consulted for painful genital ulcers that simulated herpes virus ulcers, without sexual activity for more than 6 months; accompanied by fever and odynophagia. Tests for herpes virus and trichomonas were negative but positive for Epstein-Barr virus, responding to medical treatment. The etiology of acute genital ulcer is a diagnostic challenge, due to the wide variety of possible precipitating factors and the personal and family implications that they may entail, so attempting an accurate diagnosis is extremely necessary.
