Mature cystic teratomas are benign germ cell tumors. They are the most common ovarian tumors, comprising between 10-25%. They have been shown to be bilateral about 10% of the time, with malignant transformation in 0.06% to 2% of cases. It frequently occurs asymptomatically; It usually occurs as an incidental finding in young women, with a mean age at diagnosis of 35 years. Torsion is the most common complication. On the contrary, the formation of fistulas in neighboring organs is a rare complication. A clinical case of a 31-year-old patient is described, who consulted due to chronic pelvic pain and pilimiction of 1 year’s duration. The clinical evaluation was complemented by an ultrasound, tomographic and cystoscopic study, which allowed the diagnosis of a teratoma with fistula to the bladder. Adequate surgical planning and resolution is carried out for minimal invasion.
