Resumen

Objective:
To evaluate extreme maternal morbidity in patients admitted to the intensive care unit of the “Dr. José María Vargas” maternal and child hospital, period 2023-2024.
Methods:
Retrospective, descriptive, cross-sectional study. Among 198 women admitted to the unit (167 extreme maternal morbidity, 31 deaths), a deliberate non-probabilistic sample of 59 stories was included. Results presented in frequency distribution.
Results:
Mean age 26.61 ± 6.88 years (16 - 46). Secondary education level 54.24%; marital status: couple: married 30.51%, stable union 27.12%; urban origin: 54.24%. Obstetric characteristics: II-III pregnancies: 44.07%, mean 2.57 ± 1.78 (I-VIII). There were 76.27% uncontrolled pregnancies. Comorbidities: 59.32%. Delicate entry conditions: 83.05%; antepartum care: 50.84%. Admission to the intensive care unit ≥ 24 hours: 83.05%, stay: 7.35 ± 11.99 days (1-90). Obstetric resolution: 61.08% by cesarean section. Perinatal outcomes: 83.05 % live births. Criteria World Health Organization for specific disease for extreme maternal morbidity: obstetric: 78.82% (hemorrhage: 50.74%, hypertensive disorders: 47.76%). Criteria for organ failure: hematological: 22.88%, hepatic: 18.64%, neurological: 17.80%, and genital: 16.95%. Specific management criteria: transfusion: 28.57%, vasoactive substances: 22.86%, surgical: 21.43%. Use of blood products: 221 units. Extreme maternal morbidity ratio: 20.70/1000 live births, prevalence 2.07%, maternal mortality ratio: 384/100 000, maternal mortality ratio 18.56/100, extreme maternal morbidity/maternal mortality ratio: 5.39:1.
Conclusion:
Negative indicators of extreme maternal morbidity and maternal mortality were recorded.
