The term OHVIRA (Obstructed Hemivagina and Ipsilateral Renal Anomaly) describes the association between obstructive uterine and renal anomalies. This set of malformations suggests an interruption in the sequence of events during embryogenesis, due to a failure in the development of the Müllerian ducts, causing altered fusion. The characteristic clinical presentation during adolescence is progressive pelvic pain and dysmenorrhea secondary to hematocolpos. We present three cases in which 2D and 3D ultrasound were the diagnostic confirmation method, with subsequent drainage of the obstructed side plus resection of the vaginal septum with surgical hysteroscopy, which allowed the study of the internal vaginal and uterine morphology and the definitive resolution of symptoms, respecting the hymenal and vulvar integrity. Understanding the disease has allowed us to move towards a successful minimally invasive approach.
