Pelvic organ prolapse is a relatively common condition in women over 50 years of age, associated with a deterioration in the quality of life of those who suffer from it. However, it is an extremely rare entity in young women and is associated with risk factors such as collagen diseases and anorectal malformations. We present the case of a 30-year-old nulligest patient with a history of imperforate anus, total agenesis of the coccyx, and partial agenesis of the sacrum. She was diagnosed with genital prolapse at age 9, requiring multiple surgical interventions, the last in 2023 with prosthetic material, without surgical success. Given multiple recurrences and desire for future fertility, a laparoscopic hysterosacropexy was proposed.
