We present our experience with the laparoscopic approach in 7 consecutive patients with clinical diagnosis of gastroduodenal perforated ulcer. In all the cases the technique of simple closure with interrupted stitches, omental patching, and peritoneal lavage and drainage was used. Two patients were converted to open surgery because of posterior ulcers. In the five patients completed by laparoscopy a mean operative and hospitalization stay of 80 minutes (45-120) and 5 days respectively (3-8 days) was obtained. The morbidity was of one patient who presented a purulent discharge through the abdominal drainage that was solved by this way with oral antibiotics. There was no mortality in the series. These results show that this technique represents an excellent approach for the perforated duodenal ulcer and should be the standard method of treatment in these cases.
