Aim: Treatment with 2.5 mg of Indapamide, alone or in association with other hypotensive drug, could be switched at once to 1.5 mg of Indapamide SR without affecting the effectiveness of the drug, in hypertensive (HBP) patients. Design an Methods: It was an opened study of female and male HBP patients, between 20 and 70 years with mild and moderate hypertension (WHO). Participants (43) were divided in three groups: A) New HBP patients (14) received placebo for 15 days. After that, they were given 2.5 mg of Indapamide QD for three months; at this point, if blood pressure (BP) was under control (Systolic Blood Pressure - SBP - < 140 mmHg and Diastolic Blood Pressure - DBP - < 90 mmHg, sitting and standing), they were switched to 1.5 mg of Indapamide SR QD for three more months. B) HBP patients (17) that were already under 2.5 mg of Indapamide QD therapy and under control, at least for three months, were observed for 15 days, and then treated with 1.5 mg of Indapamide SR QD for three months. C) HBP patients (12) that were already under 2.5 mg of Indapamide QD therapy in association with another hypotensive drug and under control, also for at least three months. They were followed for 15 days and then treated with 1.5 mg of Indapamide SR QD for three months more without abandoning the therapy with the other drug. All the patients had laboratory evaluations and ECGs periodically. Results: Groups: A) SBP and DBP decreased significantly after 3 months of treatment: 140.6 - 118.1 (p = 0.0020) / 93.8 - 83.6 (p = 0.0061) keeping the BP under control for three more months. B) SBP and DBP kept stable (121.4 - 127.5 / 78.4 - 78.4). C) SBP and DBP kept stable and even decreased at the last 3 months (131.2 - 121.0 / 81.6 - 78.1). BP registers were in sitting position. ECGs did not show any significant and important changes in any group. Laboratory tests showed significant changes in creatinine (Groups A and B) but under normal values, and HDL increased significantly in (Group A). Indapamide - 1.5 mg SR was well tolerated for all the participants. Conclusions: Patients under 2.5 mg of Indapamide treatment for HBP can be immediately switched to 1.5 mg of Indapamide SR keeping their BP under control, even at lower levels than before. Thus, Indapamide - 1.5 mg SR given once a day, is a very effective and well tolerated drug in HBP.
