In the treatment of the fractures of tibial diaphysis some difficulties may arise, like delay of the fracture consolidation. These have given place to controversy regarding the best approach to treat such fractures. We proposed the oral administration of Disodium Pamidronate as adjunctive therapy in the treatment of closed fractures of tibia to accelerate the consolidation. Thirty patients were randomly assigned to receive Disodium Pamidronate (PD) 200 mg OD oral for 12 weeks (Treated Group, n = 16) or a placebo (Control Group, n = 14). Controls were made in weeks 4, 8 and 12 to evaluate the clinical and radiological response to treatment. The radiological control was made with an empirical scale (grades 0, A1, A2, 81 and B2 in that order). Results: during the study, was not possible to remove any immobilization in the control group. Nevertheless, in the experimental group, during the second control, 7 immobilization were removed and walking were restored in those patients (43,75%); in the third control 4 more patients recovered their walking, for a total of 11 (68,75%) (p < 0.05 and p < 0.005 respectively). The radiological studies showed significant differences in the treated group compared with placebo in favor of the disodium pamidronate group (4 weeks p = 0.005), (8 weeks p < 0.001), (12 weeks p < 0.001). It was reported a non­ significant higher number of gastrointestinal secondary effects in the treated group. We conclude that PD is a safe and efficacious alternative as adjunctive therapy in the treatment of closed fractures of tibia.
