ABSTRACT Revision is made of the mechanism of action of inhibitors of the ciclooxygenase (COX), their efficacy and adverse effects when used alone or combined. Emphasis is made in the risk-benefit when using inhibitors of COX-2 and the clinical implications in cardiovascular physiology. We include also revision of COX isoenzymes (COX-1 y COX-2), their genetic coding, tissue expression and functions in the vascular wall. Comparison is made of mechanism of action for aspirin, non selective NSAIDs, coxibs (selective inhibitors of ciclooxygenase) and acetaminophen including clinical studies. We conclude that further clinical studies in vivo are required to study the combination of low dosage of aspirin with ANSAID, coxibs and acetaminophen, in order to determine their interactions and cardiovascular effects. We can also point out that the use of NSAIDS as drugs for the prevention of cardiovascular events has not been adequately investigated, and for that reason we recommend to carry on further investigations in this area.
