Benzydamine (BCD) is NSAID with efficiency in pathologies like pharyngitis, radiation mucosytis, posttraumatic edema and vaginal inflammation; however, studies in oral surgery are rare. It was designed a randomized, controlled, blind, and parallel study opposite to a negative control (placebo) and a positive control (Diclofenac Sodium) to evaluate oral benzydamine in the inflammatory process after third molar odontectomy. It was evaluated the edema, heat, and mandibular mobility 48 and 72 hours after the surgery. Because current methods to measure the inflammation are not precise, it was developed a computerized method to quantify objectively in cm2 the maxillofacial edema. Additionally, it was made a manual measurement with a flexible rule of the distance (mm) from the external angle of the eye to the gonidial angle, and from the tragus to the external angle of the nose. Within this method it was observed, in the group of patients treated with placebo 48 and 72 hours after the surgery, a reduction of the inflammation area that represents a spontaneous involution of the inflammatory process. Those treated with BCD and DS showed a greater reduction in the inflammatory area (p < 0.05) without a significant difference between BCD and DS in the assess time. Our results demonstrate that BCD and DS are effective to the treatment of acute inflammatory process related to the third molar extraction. Additionally, it suggests a new computerized accurate method to evaluate the maxillofacial inflammatory process.
