Extrapulmonary tuberculosis is most frequently found in pleura, lymph nodes, meninges, genito-urinary tract and the osteoarticular system. Anal or perianal tuberculosis is a rare extrapulmonary presentation, and can be found as ulcerative form (the most common), verrucous, lupoid or miliary. In this study we described the first case of double anal fissure associated to infection by Mycobacterium tuberculosis in a Cuban patient with acquired immunodeficiency syndrome with good immunological status thanks to the anti-retroviral therapy. Treatment was successful with the first-line regimen of drugs of the National Program for the Control of Tuberculosis.
