OBJECTIVE: To define the frequency of thyroid dysfunction, autoimmune thyroid pathology and nodular disease in patients with Hodgkins disease treated with radiotherapy (Rt) and chemotherapy (Ct) and to evaluate the thyroid ultrasonographic changes in these patients. PATIENTS AND METHODS: We studied 25 Hodgkins disease treated patients between 1985 to 1996. Twenty one received Rt plus Ct, 2 Rt and 2 Ct. Cervical zone was irradiated in 22 cases. Mean interval between treatment and our study was 66 months. In each patient and control we performed thyroid ultrasonography and investigated TSH, T4L and thyroid antimicrosomal antibodies. RESULTS: Eight patients (32%) showed subclinical hypothyroidism, 2 (8%) evident hypothyroidism, 3 (12%) nodular disease and 3 (12%) Hashimoto thyroiditis. Thyroid volume estimated by ultasonography showed. a significant reduction compared with controls. The heterogeneous pattern was observed in 7 of 10 hypothyroid patients (70%), stablishing a sensitive and specific method in this pathology. CONCLUSIONS: The high frequency of thyroid disease in patients with Hodgkin disease treated with Rt and Ct makes necessary the periodic and long term evaluation of thyroid function and the incorporotion of ultrasonography as a useful tool in the early diagnosis of any alteration.
