The numerous, variety, and potentially dangerous reactions and complications following an allogenic blood transfusion make necessary the knowledge about their clinical manifestations and treatment in order to identify them early and avoid their consequences. In this review, we detail all these reactions that patients who have been transfused with blood products can complain of. We focus especially in those reactions traditionally not deeply reviewed, and therefore, barely known, such as transfusion-related acute lung injury, graft-versus-host disease, platelet refractoriness, and immunomodulation. The presence of allogeneic leukocytes in transfused cellular blood products seems to be responsible for many of these reactions, by which leukodepleted blood products and gamma-irradiation are being increasingly used.
