Cerebrospinal fluid leak is a rare complication of the spinal and epidural anesthesia. We present a 33 years old multiparous woman at 33 week gestation who attended to the obstetric emergency of Hospital Universitario de Caracas, with flank pain, hyperthermia and disurya. Patient was admitted at the area with severe oligoamnios, acute pielonefritis and mild renal failure. She received a blood transfusion and antibiotics before an emergency cesarean delivery under epidural block. Six hours later patient’s reported a wet back. Liquid was tested for glucose, Lab reported 95 mg/dL; consistent with cerebrospinal fluid. The leak had high output but no clinical manifestation. 72 hours later we decided to perform a blood patch.
