Abstract

The objective of this work is to determine the feasibility of the laparoscopic approach for patients with penetrating abdominal stab wounds. The patients with abdominal stab wounds whose digital exploration under local anesthesia in emergency room was suggestive of peritoneal penetration were included. Cases with hemodynamic instability or midline laparotomies were excluded. Since june 2003 until september 2004 thirty seven patients with abdominal stab wounds were admitted, 7 of which were evaluated by laparoscopy. Peritoneal penetration was confirmed in 4 patients (57,2%) in which the findings were hepatic injury grade I in two cases, round ligament injury in one, and abdominal wall injury in the remaining. In these patients the management consisted in hemoperitoneum aspiration and peritoneal lavage. Surgical time was 60 minutes average and no morbidity nor mortality occurred. We concluded that laparoscopic approach is feasible and safe in the evaluation of the stab wound abdominal trauma with peritoneal penetration.
