The Histiocytosis X or of Cells of Langerhans it is a pathology that can show with diverse clinical squares according to the extension and the commitment multiorganic. Has little frequency in adolescent, children and young adults with an incidence of 2 for each 200 000, with more affectation in males. Their clinic is varied and of evolution uncertain. We describe the case of a feminine bigger lactate of 18 months of age who enters with fever, dermatitis progressive seborrhea, hepatoesplenomegaly, linfoadenopatias and compatible breathing affection with histiocytosis X or of cells of Langerhans, reason why they are carried out paraclínicos that allow to settle down a I diagnose and to determine the systemic commitment. The definitive diagnosis was obtained by means of the observation of granules of Birberck in the cytoplasm of the histiocytos in the study inmunohistoquimic. Treatment quimioterapic began with the help of Vimblastina and prednisona, without satisfactory answer. Patient dies to the later 45 days to diagnose.
