OBJECTIVE: We report a clinical case of malignant granular cell tumor, with special emphasis on clinical-pathological and immunohistochemical findings. ME-THODS/RESULTS: A 62-year-old female patient, who presented with a tumor in the right thigh. Local excision was performed and a 3.8 cm x 2.5 cm tumor was observed. Histopathological analysis revealed a granular cell tumor with malignancy features. The immunohistochemical study showed positive staining for S-100 protein and vimentin. CONCLUSION: Malignant granular cell tumors are extremely rare neoplasms, accounting for 1 % - 2 % of all granular cell tumors. An adequate clinical-pathologic correlation is necessary to reach a final diagnosis.
