The development of sarcomas during the course of treatment of a previous wicked lesion is uncommon. The origin of these tumours and the factors promoters of this they are controversial. There are diverse reports that they affirm the later induction of appearances of sarcomas categorically to radiotherapy administration. Cured patients of a wicked lesion have a significant probability of developing other tumours. Numerous publications they have been documented the frequency of second tumours associated with radiotherapy in patients that have survived their primary tumour. There are many examples that we can mention of carcinogenesis in human later to irradiation: Individuals that have been treated with radiotherapy for espondilitis anquilosante, use of x rays in pregnant patients, treatment of tinea capitis, use of nuclear energy in war conflicts, etc. The case that we describe is a young patient with antecedent of alveolar rabdomiosarcoma and seventeen years later; he has leiomiosarcoma radiation - induced etmoidal. We discuss their handling and the literature is revised starting from description by Cahan et al in 1948.
