OBJECTIVE: To evaluate microvascular density as prognostic factor in gastrointestinal stromal tumors compares the results with well-known clinical pathologist prognostic variables. METHODS: Prospective trial using database diagnostic gastrointestinal tumors patients at Instituto de Oncología Dr. Luis Razetti Hospital Vargas from Caracas between January 2000 to December 2004. Microvascular density was measured in high or low by inmunohistochemical biopsies, paraffin-wax molds studied about clinical pathological variables with multi variable test, observing risk factors for overall survival free survival disease, and local recurrence metastasic recurrence. RESULTS: 36 patients, 33.3 % male and 66.6 % female; median age, follow up 54.4 years 21.17 months respectively; microvascular density was high in 52.8 % low 47.2 %; local recurrence 16 (22.2 %) metastasic recurrence 14 (38.9 %), 9 patients death 9 still alive with active disease. Prognostic factors: global superlife high microvascular density (P= 0.001), tumor size > 10 cm (P= 0.039), spread disease as surgical find (P= 0.002) residual disease higher than 5 cm (P= 0.001). Free super life high MVD (P= 0.001), imagine tumor extension (P= 0.02), tumor size > 10 cm (P= 0.017), spread disease (P= 0.001) post surgical treatment with Imatinib (P= 0.036). Local recurrence high microvascular disease (P= 0.012), imagine tumor extension (P= 0.003) tumor size > 10 cm (P= 0.001). Metastasic recurrence risk factors high microvascular density (P= 0.001), imagine tumor extension (P= 0.037), tumor size > 10 cm (P= 0.014), residual disease higher than 5 cm (P= 0.003) disease progression on Imatinib treatment (P= 0.030). CONCLUSIONS: High microvascular density, tumor size higher than 10 cm are constant prognostic factors.
