To present an infrequent surgery pathology and the description of a clinical case seen and examinants in Surgical Service One of Hospital Vargas. Caracas, Venezuela. We describe a clinical case of a male patient of 80 years old to whom diagnostic a squamous carcinoma of tongue classified how T2N1M0 which do not infiltrative the mandible. We realized a glosectomy procedure of mobile tongue for a “pull-trough” technique and associated with radical right cervical dissection and cervical sections of I to a III left levels. The defect was been created was 8 cm to 6 cm size for to tape these defect we utilized a microsurgery flaps of left radial. The microsurgery flaps is a method of ideal reconstruction for old patients, how it is considered more technical complicated and produce a minor morbidity to the patient it’s demonstrated his best results.
