Carcinoma apocrine of the mammary gland anyone of its presentations infiltrate or in situ constitutes peculiarity between the epithelial tumors of the breast. Frequency in different casuistries is variable, maintaining cito-histological rigorous criteria in selectivity, where the dominant pattern is total, almost total situation by epithelia of apocrine origin; it was possible to be considered in  0.06 % to 4 % of all histological variants. Present case of patient 56 years with antecedent of axillaries mammary weave that develops infiltrate apocrin carcinoma, for its characteristics, local aggressiveness and surprising evolution, subsequent to the use neoadjuvant chemotherapy allowed total exceresis. Revision literature that allows knowing about this infrequent organization, where the inmunohysthochemical can helpful be in its identification where the positive to androgenics receivers and negative estrogen receivers and progesterone make think about different histogenic what will provide therapeutic approach, prognosis of these histological variants with uncertain histogenic of mammary carcinoma.
