Abstract

OBJECTIVE: Pelvic endometriosis characterized by presence endometrial tissue on fallopian tubes, ovaries, pelvic peritoneum; instead extra pelvic endometriosis endometrial tissues are located in vagina, vulva, cervix uterine, perineum, groin, urinary tract, gastrointestinal tract, lungs, extremities, skin central nervous system. Endometriosis tissue rarely undergoes malignant transformation; this fact occurs 0.7 % - 1 % of patients commonly is associated to ovarian disease. METHODS: We report 41 year-old female patient with an episiotomy in her delivery and a miomectomy performed at 31 year-old. She had symptoms for a year like progressive pain and tumour at the perineum and glutei area and cyclic bleeding. RESULTS: The core biopsy reported endometrioid carcinoma of perineum. She was admitted at the gynaecologic department of Hospital Vargas de Caracas with the follows diagnosis: 1. Pelvic tumour: Hematometra or ovarian tumour, 2. Solid tumour of right isquiorrectal area: endometroid carcinoma. A laparotomy was performed, the uterus was absent, a left ovarian endometrioma cyst and a right follicular cyst were detected, and appendix had endometrial tissues. Six months after the surgery the patient had liver metastases and died 2 months later. CONCLUSIONS: In the literature is defined set of three criteria used as presumptive endometriosis origin of neoplasm: endometriosis is intimately associated with cancer, histology of cancer is compatible with endometrial origin and no other primary is identified. Malignant transformation of extraovarian endometriosis is uncommon and worldwide have been reported until (1990 only 60 cases); ninety percent are endometroid carcinomas.
