Following his initial experience in Sanare with nutrition rehabilitation centers and convinced that the social reality associated with malnutrition required a larger focus, José María Bengoa was assigned the task in 1956 to evaluate for WHO the activities led in different parts of the world in nutrition. He travelled extensively through Africa, India and Latin America in order to understand the magnitude of the problem. The main topic then was kwashiorkor a syndrome of severe malnutrition due to low intake of poor quality protein. From the start his goal was to create a bridge between investigation and public health in order to manage the issues of malnutrition in the community. As Chief of Nutrition Unit in WHO from 1964 to 1975, he led assistance programs and coordinated activities with other UN Agencies leaving a widely recognized legate in public health nutrition.
