Ajoene is an organosulphur compound obtained from garlic alcoholic extracts, with demonstrated broadly antifungal activity both in vivo as in vitro. In this work we determine the susceptibility in vitro of yeast isolates of patients with onychomycosis and which were related with the in vivo activity shown by this compound. Eight patients were selected with clinical and mycological diagnostic of onychomycosis. Six received ajoene solution 0,4% and two fluconazol 150 mg/weekly for 4 months, with clinical and mycologycal controls on days 30, 60 and 90. The MIC and IC50 of ajoene, fluconazol and terbinafine were determined following the guidelines of NCCLS M27-A with modifications (RPMI 2% G and use of hemocytometer). The values of CMI oscillated among 2.34 - 70.2 µg/ml and the CI50 among 0.26 - 7.08 µg/ml. All the patients treated with ajoene showed improvement of symptoms and mycological cure after four months of treatment.
