Chronic glucocorticoid excess is accompanied by a wide range of signs and symptoms known as Cushings syndrome. This condition most commonly arises from iatrogenic causes when glucocorticoids have been used as anti-inflammatory treatment for a long time. The pathogenetic mechanism of endogenous Cushings syndrome can be divided into ACTH dependent and ACTH independent. Despite major advances in diagnosis and therapy, Cushings syndrome is frequently a challenge for the clinician. In this paper, based on levels of scientific evidence and clinical experience in the Unit of Endocrinology, IAHULA, we present the protocol for the management of Cushings syndrome, which includes: diagnostic criteria, clinical management and treatment of this condition.
