Objective: To describe the unusual case of a patient with clear cell renal carcinoma, with metastases to the pancreas and thyroid, which were initially diagnosed as isolated entities. Case Report: A 63-year-old female patient with a current illness characterized by pain in the right hypochondrium, strong intensity, permanent, without calming. She was evaluated by the surgical service, who, due to abdominal CT findings, that show a tumor of 6x4 cm in the pancreatic body and two more lessions of 1x1,5 cm in the peripancreatic area, perform a corpocaudal pancreatectomy. The biopsy reports tumor of islet cells of pancreas, with clear cells. The patient 27 years ago had a clear cell renal carcinoma and, 4 years ago, a poorly differentiated insular thyroid carcinoma with abundant clear cells. A clear cell renal cell carcinoma as primary entity with metastasis to the thyroid and pancreas is raised. Conclusions: Clear cell renal carcinoma constitutes a rare neoplasm, the most frequent invasive sites are located in lung, liver and brain, and with less frequency skin, thyroid and pancreas, reflecting a challenge in the differential diagnosis of isolated or metastatic tumor disease.
