This article aims to pass the current situation of the discussions relating to the revision of the DSM-5 and ICD-11 magazine from the American Psychiatrie Association and who in regard to what those catalogs called respectively (DSM-IV) gender dysphoria and transsexualism (ICD-11), to refer to the identities or expressions of gender which do not conform to the social construction estereotipadamente associated with one sex or the other. To this end the preparatory work performed and the current status of discussions are examined, taking into consideration the need to contribute to and Oh trans identities, faced with the evidence that the pathologization contributes to the social, cultural, educational and labour exclusion of such persons, while ensuring access to the right to health for this population. We evidenciaremos the difficulties of achieving the necessary balance, particularly in the text of the DSM, due to the nature of this compendium pertaining only to mental illness. The author touched you intervene directly or indirectly in both processes, so reflections and materials product of the discussions of the working groups that Act are used before the American Psychiatrie Association and who respectively, in so far as the duties of confidentiality does not prevent the disclosure of certain information
