An acute case of Chagas disease in a preschool male, 6 years old, from the village of Las Lajas, municipality of Sotillo, state of Anzoátegui, Venezuela, is described. Clinically on the inspection and palpation, he presented right bipalpebral erythema and oedema with a subsequent satellite node, suggesting the classical Romaña sign. He also had a flat erythematous lesion between the outer edge of the left rectus abdominis and the umbilical scar, compatible with a chagoma of inoculation. Hepatoesplenomegaly was also detected. The patient had fever of 38ºC and 39ºC. On auscultation, parasternal mesosistolic blow was heard. As treatment with Clindamycin was not resolving, we decided to investigate the presence of causal agent in blood, by means of direct and indirect methods, serology and molecular techniques. The parasitological study demonstrated the presence of Trypanosoma cruzi, which showed moderate virulence in blood, murine models, with broad spectrum of colonized organs, especially cardiac, skeletal, smooth muscle cells and atypical invasion of kidneys. The molecular characterization of isolates showed a pattern compatible with TcI lineage, the most distributed in Venezuela. These results indicate that active transmission of Chagas disease in the state of Anzoátegui persists and more investigations are required.
