Organ donation and transplantation is a crucial therapeutic achievement to save lives. The study adopted a qualitative approach with a descriptive scope, developing a Systematic Review (SR) following the PRISMA 2020 guidelines. The objective was to analyze international transplant activity through a systematic process of searching, analyzing, and interpreting secondary data. The methodology involved selecting articles from databases such as Web of Science, Scopus, SciELO, Pubmed, and Google Scholar, published between 2020-2023. From 84 initial articles, 18 research works were selected after applying inclusion criteria. Results revealed a global shortage of organs for transplantation, with obstacles such as limited political commitment, insufficient funding, and inadequate resources. Donation rates varied significantly among countries. It is concluded that optimizing access to transplants requires comprehensive management that simultaneously addresses medical, educational, legal, and cultural aspects.
