Transplant activity requires significant resources for effective operation, however, it faces important budgetary challenges that limit its development. This research aims to analyze the necessary budgetary reform to develop transplant activity. A qualitative, documentary, and descriptive study was conducted through a systematic review of 19 scientific articles published between 2014-2024, selected using the PRISMA checklist. Results show that health budget allocation is insufficient to cover operational costs, directly affecting transplant development. The analysis revealed that 53% of studies focus on budgetary reform and 26% on financial management, highlighting the need to transform current financing mechanisms. It is concluded that legislators must implement comprehensive budgetary reform that optimizes resource allocation, strengthens financial management, and ensures transplant program sustainability, thus promoting greater equity in access to these specialized health services.
